Once you have an appointment in this office and to help us make your initial visit go smoothly, we ask that you please read and follow the instructions below:
- Please complete the patient registration form and medical history form included at our site.
- If your insurance plan requires a referral authorization form, it is necessary that you call your primary medical doctor to arrange for them to issue a referral. Without this prior approval of the referral, your insurance will not cover the cost of this visit. You will then become responsible for all charges. The primary medical doctor may fax a copy of the referral to our office at 203-401-4687; mail the referral to our office at 40 Temple Street, Suite 4A, New Haven, CT 06510; or you may bring the referral with you at the time of your visit.
- Many insurance plans require the patient to pay a co-pay for each visit. We will ask that you see our insurance coordinator before you see the doctor on the day of your visit. Payment must be made at this time.
Please arrange to arrive fifteen minutes prior to your appointment time so that any paper work may be processed before your visit. We will make every effort for you to be seen at your scheduled time. To expedite your visit, it is important that you complete the patient registration form, the patient medical history form and that you bring your current health insurance cards, as well as your co-pay and a referral from your primary medical doctor if applicable.
